In Nashville, Tennessee, the Park Center and Metropolitan Homelessness Commission has pledged to use the findings from 320 Vulnerability Index Surveys to drive their planning process.

Since 1984, Park Center, a nonprofit in Nashville, has helped people with mental illness achieve their full potential and gain respect as co-workers, neighbors and friends. In 2005, Park Center partnered with the Metropolitan Homelessness Commission to expedite applications for Social Security disability benefits (SSI/SSDI) for chronically homeless individuals diagnosed with a mental illness. Now, more than 100 disabled individuals experiencing homelessness have a stable income and health insurance that help lead to recovery, housing and stability. To better inform local planning and system coordination efforts, Park Center and the Metropolitan Homelessness Commission have partnered with Common Ground National to implement Common Ground’s Vulnerability Index.
